Synovial sarcoma accounts for 5 to 10% of soft tissue sarcoma. It occurs mainly in young adults, where 90% of cases occur before 50 years. Over 80% of cases arise in deep soft tissue of extremities, especially around the joints [1]. Unusual sites (about 15%) have been reported, including head, neck and trunk where differential diagnosis is difficult and surgical treatment is complex due to proximity to vital structures. Here, we report a case of synovial sarcoma in an unusual site which is thyroid gland with special clinical, histopathological, immunohistochemical and molecular features.

Background: It has been estimated that over 400,000 patients die each year from medical errors in the United States. The purpose of this study was to investigate the impact of vitamin D (a steroid hormone and immune modulator) and glutamine supplementation, evidence- based medicine, and close staff supervision on hospital mortality rate in trauma patients. We hypothesized that vitamin D and glutamine supplementation, evidence-based medicine, and close staff supervision has an appreciable or measureable effect in reducing the hospital mortality rate in critically injured patients admitted to surgical intensive care units and to surgical floors.

Presently, the prevalence of congestive heart failure in the United States is 6.5 million people. We report two cases of severe congestive heart failure patients with a very poor prognosis, treated successfully with a combination of high dose vitamin D, digoxin, and BiDil (Matthews’ Two Inotrope Protocol). Further studies are needed to fully appreciate the overall healthcare changes associated with this simple drug therapy.

Asthma is a chronic lung disease. The prevalence of asthma in pregnancy is 1-4%. Asthma can have a negative impact on maternal and fetal health. But it depends if treated adequately or not, during pregnancy. A high risk exists that asthma be complicated before and during delivery, but after delivery, it improves very soon. If asthma worsens during pregnancy, the increase in symptoms usually happens during week 24 to week 36 of pregnancy. If asthma was treated effectively during pregnancy, it can have little or no risk for complications. But, if asthma was severe or poorly controlled during pregnancy, can occur more complications, including: nausea and vomiting, vaginal bleeding, premature birth, pre-eclampsia, intrauterine growth restriction (IUGR), low birth weight, fetus with congenital malformations, complicated labor and neonatal hypoglycemia, tachypnea etc. There are many etiologic factors, that could cause asthma, including; allergens, dust mites, animal dander, etc. Then irritants as cigarette smoke, air pollution, and chemicals. Also, sinusitis, rhinitis, some medications as non-steroidal anti-inflammatory drugs (ibuprofen and naproxen), aspirin, beta blockers and others factors, including emotional stress and cold air. In our case, because of asthma the poorly controlled during pregnancy have happened complications as, preterm birth, PPROM, intrauterine growth restriction (IUGR), low birth weight and the need for neonatal intensive care.

Background: To investigate the clinical effect of Nd:YAG laser combined with total glucosides of paeony (TGP) for the treatment of erosive oral lichen planus (OLP). Study Design: Sixty patients diagnosed with erosive OLP with clinical symptoms were divided into an observation group (n=28) and a control group (n=32) using a random number table. All erosive OLP patients received TGP while only the patients in observation group subjected to Nd:YAG laser. The clinical effects were evaluated comparatively three months before and after the treatment.

Background: Socioeconomics only account for 18% of all healthcare disparities. Healthcare disparities in the intensive care unit (ICU) have been well documented and persist in spite of previous government and medical interventions. Vitamin D deficiency is the most common nutritional deficiency in the United States and the world. This deficiency has been largely overlooked in the debate on healthcare disparities.

We report the rare case of a left isolated superior rectus muscle (SRM) rupture due to a bizarre mechanism of injury, where a universal serial bus (USB) flash drive entered the orbit during a motor vehicle accident leading to rupture of the SRM 7mm from the limbus. The proximal stump was relocated and reattached using a non-absorbable hangback suture. Post-operatively the patient had a residual 5 prism diopter left hypotropia (LHoT), which allowed for fusion in primary, with a slight chin up position, as well as downgaze. The globe, eyelids and orbit were spared of injury and orbital fractures were absent. We discuss our experience and surgical management of the case, which included locating the lost muscle and repairing it primarily in preference to a transposition.

Background: Screening programs for detecting cancer early are critically important for a better prognosis and a long acting survival. In our country, second most common cause of death is cancers with the rate of 21.1 %. The aim of this study was to investigate cancer screening and awareness in healthy individuals in our region.

The article outlines the principal theses of the concept of psychotherapy of anxiety disorders, based on changing the client’s perception on his problem and revising the system of values. Model of “anxiety-centered” behavior is reviewed and ways of transformation of client’s self-evaluation and lifestyle that are to manage the anxiety are described. Changing the “reference system” as core element of recovery is reviewed. Material is illustrated with case studies and excerpts from therapeutic sessions.
